FLOW OF PATIENTS ADMISSION TO DISCHARGE
a. Patient or relative will go to the information for data collection to be encoded at the Hospital Information Management System
b. The Admitting staff will provide information on the type of rooms you can choose from ward, semi private to suite room. You can also secure claim forms for Philhealth and Health Insurance (HMO) with a list of requirements you will need to process your claims.
c. For Patient with Health Insurance accredited to the hospital, to avoid inconvenience please present your HMO and company ID upon admission for verification.
d. The PMMGMPC Cooperative Hospital encourage that all medication and supplies be obtained from the hospital pharmacy.
e. The nurse will inform you of schedules and procedures you will undergo as well as the appropriate preparation you needed. They will also inform you of any instructions your attending physicians might give. You will also be given a patients satisfaction survey that will be collected upon your discharge.
f. Meals are provided by the dietary service as prescribed by your doctor and prepared under the supervision of our nutritionist dietician. The food tray will be collected an hour after the delivery
g. Our Costumer Relation Officer will provide you with a partial running bill during your confinement or if you wish to have an updated copy of your bill you can proceed to the Costumer relation service at the hospital lobby.
h. Please make partial arrangements with the cashier once you received your bill to avoid interruptions of services to your patient.
i. Your attending physician will tell you when you are well enough to go home. On his order your hospital bill will be processed.
j. Please wait in your room while your bill is being processed. The billing clerk will call the nurse station requesting to inform you that your final bill is ready for release at the Billing Section and to sign the documents for Philhealth claims located at the ground floor of the hospital. The Billing and Philhealth Department is open from 7:00 am to 7:00 pm daily.
k. Please check your bill thoroughly before paying. Payments are accepted only at the Cashiers Office at the entrance area beside the information. Please do not give money to staff and strangers who may come to your room.
l. After you have settled your account, you will be given 2 discharge slips which you will need to present to the nurse on duty who will give you your take home instructions based on your doctor’s advice and recommendations, list of medicines you will need to take at home and the date of follow up checkup. Your nurse will also collect the accomplished patient’s satisfaction survey.
m. The second copy of discharge slip will be given to the guard on duty along Mabini entrance area. Our attendants will also do the final check of your room and facilitate your actual discharge from the hospital.
General Reminder: For your safety and protection
1. Please do not leave your things unattended.
2. Please don’t bring large amount of cash, jewelry or other valuables to the hospital. The hospital will not be liable for any loss of personal items or valuables
3. Smoking is strictly prohibited inside the hospital premises
4. Do not allow strangers, unidentified personnel into your rooms. Hospital personnel on duty will be identified with their hospital ID and uniforms which they are required to wear all times.
5. Visiting Hour: 9:00 am to 9:00pm
6. Children below 7 years old are discouraged from visiting the hospital to avoid exposing them to deceases and infections.
7. Because of high rates of intentional damage and vandalism, rooms will be checked prior to discharge. Any loss/damage will be charge to your account
8. Remote controls for TV and aircon may be obtained from the nurses’ stations. Please surrender it to the nurses’ station before discharge to the nurse on duty signs your clearance.
9. To help our environment and follow the City Rules. Please throw your trash in an appropriate labeled trash can.
10. Bringing of portable electrical appliance is discouraged
11. Observe silence. Our patients need peace and quiet to rest and recuperate. They will surely appreciate your cooperation.
